Welsh Government Social Prescribing Pilot in our Cluster

This project is open to all patients living in our Taff Ely cluster area and registered with any of the Cluster practices (with the exception of Ashgrove and Parc Canol who are not taking part at this time).  It is being run by the mental health charity - Merthyr and the Valleys MIND.
 
The project aims to help patients managing 'low level' mental health issues, including general social problems they are facing in life, housing concerns, financial stress, and isolation.
 
Participants must be over 18, and be able to give written, informed consent to taking part in the study.

Practices in the cluster will now hold Patient Packs at all of our receptions, for patients who request one and who have completed the self referral form (available here) which your practice will then forward on to MIND.  

This form does not need a GP/staff signature.   Once received by MIND, a link worker will contact you within 5 days, so please read through your patient back before then.
 
We hope that allowing patient self-referrals will allow this project to now be more accessible to patients.  See below for patient leaflet on this service.
