Frailty Nursing Service
The Frailty Nurses are working for the GP practices in Taff Ely to provide assessment, advice and support for patients, their families and carers at home.
The nurses are there to work with patients and their families to work out what they want to do to manage a patient’s condition effectively at home, with an aim to allow them to live as well and independently at home as possible. They will help plan and work together to avoid the need for unnecessary hospital admissions or A&E attendances.
They also work closely with other healthcare professionals and agencies such as, social care and voluntary groups to provide patient care and support.
Who is it for?
The Frailty Nursing service is for people:
- not already regularly supported by other services
- 65+
- registered with a GP practice within Taf Locality
What to expect
Frailty Nurses will discuss what is important to you and help to find ways to maintain your health and well-being.
They provide a co-ordinated approach designed to help with any medical conditions, mental health, functional capacity and social concerns you may have.
They can provide an overview assessment and help navigate to the best available sources of support for you.
They can complete physical checks, arrange rehabilitation or plan for long term follow-up.
During a home visit or telephone call, they will focus on what is a concern for you. For example, if you are experiencing reduced mobility, weight loss, generally feeling ‘slowed up’ or becoming more reliant on others.

Monday – Friday 9am to 5pm. If they are unavailable, you can leave a message and one of the nurses will get back to you on their next working day. This is a non-urgent service.
For anything urgent, you should contact Telephone First 111 or your GP practice. If it is an emergency, you should call 999.

Case UK Wellbeing Support
Case UK offer rapid access to physiotherapy, psychological, and occupational therapy across Cwm Taf Morgannwg and Taff Ely area. It is fully funded by the Welsh Government and comes at no cost to you.
