Cluster Work Summary
Welsh Government encourage GP practices, Optometrist, Pharmacists, Dentists to work more closely as a cluster to provide you with your primary care in your community. We work together with the voluntary sector and social care, to ensure patients have access to the right services and support.
There are many areas in which the Primary Care clusters work together, including, but not limited to:
We work together to review good practice to:
All frontline reception staff in GP practices have been trained so that they have additional skills and knowledge and information to signpost and help patients get the right support and see the right person and at the right time.
We will continue to developing other projects and work together to address our population health needs and will keep our patient population up to date on this website and our facebook and twitter pages over the coming months and years.

- Reviewing current practice and services
- Improve access to specialists closer to home
- Employment of pharmacists, mental health practitioners, physiotherapists to work within practices to ensure that the right specialist advice and support is available to you.
- Support access to the most appropriate services in the community
- Investing in equipment and resources to support delivery of services
We work together to review good practice to:
- improve cancer diagnoses
- support patients with their care plans and help to manage their conditions
- support end of life care
- reduce risks of polypharmacy (when patients take multiple medications)
- improve access to specialists closer to home
