Taff Ely

The service supports people over 65 years to remain as independent or as well as possible in their chosen place of care. The Nurses provide proactive care working alongside GP practices.

Who can benefit

As we get older we may feel like we are ‘slowing down’. This can often be mistakenly seen as an inevitable part of aging. However, sometimes we can suggest things to slow this process down or improve symptoms.

  • Are you over 65 years old?
  • In general do you have any health problems that require you to limit your  activities?
  • In case of need, can you count on someone close to you?
 
We want to ensure all our patients registered with our GP services in Taff Ely receive the best possible healthcare. This includes looking for problems early before they develop into something more serious.
 

​How we can help

​A coordinated approach designed to help with any medical conditions, difficulties with daily activities, mental health and social concerns you or your family may have.
We can provide an overall assessment and help navigate you to the best available sources of support. We complete physical checks, arrange rehabilitation as well as helping plan for future health and care needs. During a home visit or telephone call, we focus on what matters most to the person.

​Assessment: What matters most to you?

Providing nursing assessment and care planning for falls prevention, medication support, management of symptoms, nutritional concerns, skin care, continence, memory, mood, social or home care needs.
 
We can provide follow up support or signpost to the most appropriate resource to help you.
